  • What insurance do you accept?

    We will accept most major commercial insurances PPO/HMO along with some government issue insurances. If you do not have insurance we take self-pay patients. (Patients are responsible to know if their insurance is in network.) List is pending final insurance contracts and press "Manage Questions" button.

  • Cancellation Policy

    If you need to cancel or reschedule an appointment, please call 24 hours prior to your scheduled appointment. If not cancelled within 24 hours of your schedule appointment, there will be a $25 cancellation fee that will need to be paid at the time of next appointment. This is so we can accommodate other patients that are waiting to be seen. If you arrive 15 minutes after your scheduled appointment you will be rescheduled and charged $25.

  • How do I get a refill?

    Please contact your pharmacy One (1) week before you run out of medications. Please allow up to 2 business days to receive your refills. All maintenance medications (i.e. blood pressure, diabetes, cholesterol etc are to be seen in office every 3-6 months) All controlled substance medications (i.e. pain medications, ADD/ADHD, sleep aids are to be seen in office monthly)

  • How Do I request Medical records?

    You may come into the clinic to file out the proper form or download the medical release or obtaining medical records forms. If you are requesting your records to or from doctor to doctor there is not fee. If you are requesting medical records for yourself or non-doctor to doctor there is a $25 fee up to the first 20 pages and 15 cents for any additional pages. All records can take up to 15 business days (Texas Law)

  • What is the patient portal and how do I sign up?

    The patient portal is how you can communicate with your doctor/nurse, request appointments, review imaging and lab results. You can sign up on the website or we can help setup the patient portal at your next doctor’s appointment.

  • What do I bring to my appointments?

    At your appointments please bring in your ID, Insurance card and your medications so we can accurately determine the dosage and frequency. You may also bring in any previous lab results, imaging results or previous provider notes. Please arrive at least Fifteen minutes before your appointment, to fill out new patient forms and collect insurance information.

  • How do I pay a bill?

    We only accept payments for PrimeCare statements. Please click on this link: to make a payment, or you may make a payment in person or by Phone. We do not accept any payments for third parties (i.e. Quest or imaging facilities)

  • What services do you offer?

    We are a family practice we see all ages; we offer annual checks for pediatrics and adults. Vaccines and CDC recommended injections. Birth control oral and/or uterine devices and Nexplanon. Acute and chronic diseases (i.e. Ear Infections, Bronchitis, Asthma, Diabetes, Gastritis’s, Gout, Laceration repairs) Pap Smears, Cryo therapy, DOT physicals, Middle and High School sports physicals.   • Who do I call during after-hours?

    You may call the office number, there is an answering service that will take all messages, we will respond by the next business day either through patient portal or phone. If you ask for the doctor to be paged for non-emergency related concerns, there will be a $50 charge. Medication refill is not an emergency, please contact your pharmacy for any medication refills.

  • How do I schedule an appointment?

    You can schedule an appointment through the patient portal and by calling the office 817-962-0056

  • What method of payment do you offer?

    All major credit cards, cash, apple pay. We do not accept personal checks.

  • Confidentiality

    At PrimeCare Family Practice we have a strict patient confidentiality policy. Your medical record is strictly private and will not be shared with your employer, friends, or relatives without your written permission.

  • Do I need a referral to be seen?

    No, we are a primary care physicians office. If your insurance is a PPO (no physician required) please call our office and schedule an appointment. If your insurance is an HMO, please contact your insurance and change your PCP to our office PrimeCare. We are not specialists, if you need to see a specialist, we can refer you to one.
